Am I eligible and how much does it cost?
Anyone can purchase Healthy Kids coverage for the full price of $98.00 per child per month. For low-income families, the amount of your premium is based on your household size and monthly income. Most families pay either $15 or $20 per family per month. A family of 4 people is considered low-income if they earn less than $36,400 per year. How do I apply?
Enrollment is now open year round. All you need to do is fill out a simple application form, attach income documentation and send it in. During the open enrollment period applications may either be downloaded from www.healthykids.org or you may obtain one by calling 888-540-KIDS (888-540-5437) and one will be sent to you.
